This can be used as an exit slip or to get feedback from students.  Coming from a baseball background I like it when students are active.

At the end of class on scrap paper (no name) students answer a question, share something they are confused about, etc.  After they write it, they crumble it up and throw it to me, and I catch it in my baseball glove.

This can be adapted to other sports as well such as, basketball, football, etc.
